Treatment: We may use your health information to provide you with medical/dental treatment or services. We may disclose health information about you to physicians, technicians or other office personnel who are involved in your medical care and treatment.

Appointment Reminders: We may use and disclose your health information in connection with our efforts to remind you that you have an appointment or give you information about treatment alternatives or other health care services that may be of interest to you.
